About the Opportunity

We are looking for qualified professionals to join the panel of intermediaries in Queensland.

Intermediaries are based in areas that fall in the catchment of Brisbane and Cairns District and Supreme Courts (refer maps).

If you have skills in engaging with children, people with intellectual, cognitive or physical impairment impacting communication, people with mental illness or who have suffered trauma, and people who have difficulty communicating, please consider applying to be an intermediary.

If you are interested in improving access to justice for vulnerable witnesses, please consider applying to become an intermediary.

While there is no guarantee of work, you will be paid for your services (refer Schedule of Fees).

What we are looking for

  • Contribute to change - Evidence from the longest running intermediary scheme in the UK indicates there has been a cultural shift in the way all stakeholders in the justice system approach and treat victims of crime who experience communication difficulties.

Payment for services

  • Get paid for your services – this is not voluntary work.
  • Intermediaries will be paid for each engagement (there may be several engagements for each stage) according to the payment included in the Schedule of Fees.

Flexibility

  • You can continue to undertake your primary job.
  • Choose when you are available.

What is the process for inclusion to the panel of intermediaries?

Applications will be assessed as needed throughout the life of the pilot program (through to June 2023).

Suitable individuals will be invited to undertake competency-based training.  These individuals must successfully complete the training before they can be appointed to the panel. 

The process from the time you apply to become an intermediary to the time you become an intermediary depends on a range of factors, but you will be kept informed of the process.

The Transition Care Program (TCP) is a major policy initiative between the State and Commonwealth Governments.

The TCP aims to minimise the number of older people experiencing inappropriate extended hospital lengths of stay, being prematurely admitted to residential care and/or prevent unnecessary presentations or admission to the Emergency Department. The program provides short term support and active management for older people at the interface of the acute/subacute and aged care (i.e. Community and residential) sector. The program is goal orientated to optimise the patient’s functional capacity and to finalise and access their longer term care arrangements or maintenance at home.

The TCP can be provided as a bed based or as a home based service. By offering case management, low intensity therapy and personal support, TCP allows older people more time and support in a non-hospital environment to complete their restorative processes, optimise their functional capacity and finalise their long term care arrangements.    

About the Role

The TCP Occupational Therapist will work closely and collaboratively with the interdisciplinary team, the Senior Clinicians and the TCP Manager to provide clinical care to the TCP clients across both the bed-based and home-based settings.

The Occupational Therapist will work closely with the local team to deliver quality services to clients through strong clinical expertise. The acts as a resource person on Occupational Therapy matters to other team members. The position aims to provide a flexible and responsive service that works to meet each client’s identified Transition Care goals. The Occupational Therapist has extensive knowledge of community services and the residential aged care sector and has highly developed skills in working with carers and families. The position involves working across TCP’s Home Based/Community Program and the Bed Based Program.

About You:

About the Role

There are two components to this dynamic, varied role - complex care occupational therapy and complex care coordination.

Health Independence Program, Complex Care Services

This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning. SVHM HIP consists of integrated hub sites across several locations including Fitzroy, Kew, North Richmond and Fairfield.

This position will be part of the HIP Complex Care Service (CCS) Complex Aged Team. This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP Complex Aged Care manager and will take into consideration appropriate skills and training.

The key aim of Complex Care Service is to improve health outcomes to people who either frequently present, or who are at risk of presenting to hospital, because of complex medical and or social issues. CCS currently consists of multidisciplinary teams based at SVHM Fitzroy, Fairfield, and at NRCH Hub. Clinical service delivery is undertaken in the clients’ own home and their local community.
